What is FOR code and what are the possible scenarios where it can be inserted in a pilots’ roster?

The FOR Code stands for FLIGHT OPS REVIEW and is inserted into the system by the scheduler: (a)    In case a pilot refuses an assignment which he/she has been scheduled for in his/her planned roster. (b)    In case a pilot refuses his assignment during his reserve period, and the assignment complies with the FDTL/ company norms and is within the framework of Pilot Employment Agreement.  (c)    In case a pilot is not contactable to confirm his/her duty assignment. (d) In case a pilot does not report for his/her confirmed/assigned duty. (e) In case the pilot reports sick at short notice or on intimation of his/her duty assignment.  (f) Any other instance which requires a review from the Flight operations team. Such refusal of assignments is referred to Line Operations for review. FOR cases would be enquired into by a member of the Flight Ops Leadership Team, and after considering all aspects of the episode, (including a review of recorded phone conversations, if applicable), appropriate action is taken to obviate recurrence. In certain cases, where it appears that a pilot has been in the wrong, an Administrative action as deemed necessary by Chief Pilot (Line Ops) would be taken. At the same time, if it is established that the actions of the Pilot were justifiable and the issue raised by Scheduling is an error, then, appropriate action is initiated with scheduling and the FOR code shall be removed.
